Social Prescribers

Social Prescribing provides GPs and Healthcare staff with a non-medical referral option that operates alongside existing treatments to improve a patient's health and wellbeing. 

Social prescribing starts with a conversation, this may be with a GP or someone else within the practice team. Link workers support people with the social, emotional, or practical issues they may be experiencing. This could range from social isolation, housing issues, problems finding employment and much more.

Our team link closely with our Care Co-ordinators, practice colleagues, voluntary sector and community organisations to ensure patients receive the most appropriate support to improve their wellbeing.
