Care Co-ordinators

Our team of practice-based Care Co-ordinators aim to bring together all of a persons care and support needs and explore options to meet these with a single personalised care and support plan (PCSP). Support varies from help making appointments, providing good quality verbal or written information to help people make good choices about their care, liaising with Social Prescribers, practice staff and voluntary care organisations and help to increase people's skills and confidence in managing their own health.
